Submission Requirements:

We will work the first completed submission that comes in and only release a quote to the broker of record for fleets 11 units and over.  A complete submission should consist of:

  • Transportation Specific Application (No Acords) that contains historical and projected information for the current and prior three years. (Unit Count, Mileage, Revenue)
  • Current company run loss runs dated within 90 days of the effective date for the current and prior three years.
  • Equipment Schedule –  Preferably in excel containing year, make, model, vin, and stated values for any unit that requires physical damage
  • Narrative of operations
  • Expiring pricing

With the above information we will determine if the account fits our current appetite and respond to you with our direction.  If we plan to move forward and release a formal quote we will require the following additional information:

  • Drivers List – Preferably in excel consisting of Drivers name, Date of Birth, Date of Hire, Years of experience, and State that they are licensed in
  • MVRs for all drivers run within 90 days of the proposed effective date
  • For carriers that file IFTAs we will need the schedule B’s for the last 4 quarters
  • Provide detailed claims notes for losses over $50,000 and any open losses over $25,000 in the current year
